Credit Card Policy
A credit card is required to book an appointment and guarantee your appointment time. Your card will not charged until you are in violation of the cancellation policy. The information left on the online scheduler is SSL protected, you and I are the only people with the capability to view your information. At the time of your appointment, you are welcome to pay via cash, check, or other credit card.
Cancellation Policy
A 24 hour notice is required for any cancellations or changes to your appointment. If you are not able to comply to the 24 hour policy, the credit card number given at the time you scheduled/requested your appointment will be charged the following stated fees: If less than 24 hours cancellation/changes, you will be charged 50% of your booked appointment. No-Shows will be charged 100% of the scheduled service(s). If I must cancel your appointment, in less than the 24 hour policy, your rescheduled appointment (for the same service) will be discounted 50%.
Contraindications to Treatments
I am dedicated to providing you the highest quality treatments and service available. Possible contraindications to spa treatments include, but are not limited to pregnancy, high blood pressure, infectious skin disease, epilepsy, asthma, and use of excessive daily medications or other drugs or alcohol. If you are pregnant or experiencing any other medical condition, it is imperative that you inform me at the time of booking.
Insurance
Individual Kneads does not accept medical insurance as form of payment. I understand that some insurance companies do accept massage as a medical benefit, as they should. However, the reality is that most do not. Therefore, I cannot bill your insurance provider for your services. However, per your request, I can provide your with a receipt or billing statement so that you can turn in a claim to your insurance provider.
Under Age Clients
Clients under the age of 18 must have a parent or guardian approval of treatment. This approval must be done in person, therefore, this person must be present during the first appointment consultation. At that time, treatment and conditions can be discussed.
Gift Certificates
Discounts will not be applied to the purchase of or the redemption of gift certificates. Any remaing gift certificate value left over after your service is complete, will be given as a new gift certificate. Gift certificates cannot be exchanged for cash. Gift certificates are non-returnable.
Non-Gold's Gym Members
Individual Kneads is open to the public. Gold's Gym Membership is not required to visit Individual Kneads. However, clients must sign in at the front desk as a written statement of entry.
Release of Liability
You must sign the release of liability during your first consultation. Individual Kneads/Lisa Silvers is an Independent Contractor that carries it's own individual liability insurance. Assume any and all risk associated with undergoing any treatment, and fully release Gold's Gym and the McCall Gym Group from any injury, harm, damages or liabilities sustained as a result of undergoing any treatment with Individual Kneads.
Gratuities
Gratuities are not included in treatment prices. Rewarding your therapist for excellent service is at your discretion and is greatly appreciated.
